Online Farm to School quiz yes or no questions

1. We have identified school and community members that will be active in the creation of a farm to school program.

2. We have reviewed my School’s Wellness policy to ensure Farm to School is in line with our School goals.

3. Our food service staff is in full support of the creation of a Farm to School Program.

4. Educators in our school are in full support of the creation of a Farm to School Program.

5. We have assessed our school or school district’s needs around the creation of a Farm to School Program.

6. Our school administrators are in strong support of integrating local foods into the cafeteria, classrooms and community.

7. Our food service staff has experience with purchasing and preparing local foods?

8. Our classroom teachers integrate food or farming education in their classrooms?

9. Our school has created farmer relationships, either through food service staff or educators?

10. Our school is active in community outreach through one or more of the following: newsletters, press releases, web site or community events.

11. We have identified a person on staff that has the time and ability to coordinate Farm to School efforts.

12. We have identified community resources that will support the creation of a Farm to School Program.

To get to implementation you must have #’s 1, 2, 3, 4, 6, 9, 11 & 12

To get to planning you must have 1, 3, 4, & 6

Preplanning if no to 1,3, 4 & 6 go back to the drawing board.
